  • Medical Director

    In addition to ensuring that patients are provided with quality end-of-life care, the hospice medical director also plays a crucial role in supporting the hospice care team and ensuring that every facet of the hospice’s patient care program is operating at the highest of standards.

  • Nursing

    The patient is assigned a case manager nurse, who typically visits one to three days a week. All patients will also have access to an on-call nurse who is available 24 hours a day.

  • Home Health Aide

    Our aides assist our patients with their personal care and typically visit a few times a week.

  • Durable Medical Equipment

    We provide the equipment necessary to provide a safe, comfortable, caring environment in the home. These supplies might include, for example, a hospital bed, a wheelchair, oxygen, adult diapers, bandages, and latex gloves.

  • Pharmacy

    All medications are typically covered that are related to the hospice diagnosis and those intended to control or alleviate pain and symptoms.

  • Physical Therapy

    If appropriate, our hospice will provide a physical, occupational, and/or speech therapist.

  • Occupational Therapy

    If appropriate, our hospice will provide a physical, occupational, and/or speech therapist.

  • Speech Therapy

    If appropriate, our hospice will provide a physical, occupational, and/or speech therapist.

  • Volunteer Services

    Our hospice team has a group of amazing volunteers that assist the staff, patients, and their families with whatever they may need.

  • Bereavement Care

    Bereavement counseling is provided for the patients and their families. This care also extends one year after the passing of the loved one.

  • Crisis Care

    Our teams are equipped to handle an assortment of patient crises in the home. We have a 24 hour dedicated on-call team who can provide care day or night.

  • Inpatient Respite Care

    This form of temporary, short-term assistance can help alleviate or avoid caregiver burnout and stress.

  • Spiritual Care

    Our chaplain is available to patients and their families in need of spiritual support during this difficult time.

  • Medical Social Worker

    Patients are assigned a social worker to assist them with their emotional and/ or social needs.

  • Wound Care

    In seeking to provide patients with the highest quality of life, wound care is a big part of that. Our patients deserve to have symptoms of all types managed, including wound management.
